Bachelor of Science – Prelicensure Program

Instructions for Freshman Applicants Only: There are two separate applications to complete at two separate sites.

Deadlines: November 30, 2023 and January 15, 2024

November 30, 2023 is the deadline for all freshman applicants to the Prelicensure Bachelor of Science degree program to complete a UC Application for Freshman Admission and Scholarships 2024-2025 (http://admission.universityofcalifornia.edu/)

January 15, 2024 is the deadline for the UCLA School of Nursing – Bachelor of Science - Supplemental Application for Freshman Admission and all supporting documents, this site (https://apply.nursing.ucla.edu/)

January 25, 2024 is the deadline for your Recommenders to submit their online Recommendation Form

General Instructions for the Supplemental Application:

This site contains a form you will need to complete. Please allow yourself plenty of time to complete this form. You may log into your application as many times as needed to complete. These instructions may be printed, use the Print Instructions at the top right of the page.

• The form is where you provide supporting documents and Recommenders.
• The form has Save and Check Form buttons. You should Save often. The Check Form button checks that each field is filled. It does not check that the data you entered in each field is accurate and correct. It is your responsibility to review the data you entered to insure that your data is accurate and correct.

To start, complete, or change a Recommender, click the Continue button below.

To finish, you must click both the Submit button and the following OK button. Once submitted, there will be an on-screen confirmation. Your Recommenders will be E-Mailed your request for completion of the online Recommendation Form. You may login later to (1) review your Recommender's completion of their Recommendation Form, (2) send a reminder to your Recommender(s), or (3) change your Recommender(s).

Form Specific Instructions:

Required fields are marked with a red asterisk.

Prepare your supporting documents in .pdf file type. We suggest you select a font that is generic and easy to read, i.e. Times New Roman.

Your supporting documents include:

• Work/Volunteer Experience in résumé format
• Statement of Purpose

• Your Work/Volunteer Experience, in a résumé should list all employment, volunteer and/or extracurricular activities in chronological order, beginning with the most recent. Include dates of employment or participation and amount of hours spent each week or for each event, a brief description of the position or activity, name of company or agency with city and state.
• Your Statement of Purpose should be 2 - 3 pages and double spaced.

• You should include information indicating:

• your own assessment of your potential for undergraduate study and for a professional nursing career
• your reasons for selecting the UCLA School of Nursing
• your multicultural experiences including dates of involvement and amount of hours spent each week or for each event
• your bilingual abilities including how you became bilingual, or examples of your use of your bilingualism
• your economic, educational, or social disadvantages
• your volunteer, work, or other life experiences including dates and amount of hours spent each week or for each event, and how these influenced your decision to pursue a nursing career

• You may include information indicating:

• any specific personal life challenges you have experienced

Remember to upload your supporting documents as .pdf file type. After you upload, the site will display the file name. You may only replace the file before you Submit.

Arrange for TWO personal Recommenders. Appropriate sources include a teacher, a counselor, or a professional who has had supervisory, educational, or mentoring relationship to you. You will need your Recommenders: Name, Institution, E-Mail.

• High school transcripts.

Application Privacy Notice: The California Information Privacy Act requires the University to list the following information for individuals who are asked to provide information about themselves:
1. The principal purpose for requesting the information is to consider your application for admission to the UCLA School of Nursing.
2. This information is solicited in accordance with University policy, adopted pursuant to Article IX, Section 9 of the California Constitution.
3. Failure to furnish each item of information will delay or may prevent completion of the purpose for which this supplemental application was intended.
4. Information furnished in this supplemental application may be used by various University departments and may be transmitted to State and Federal government agencies if required by law.
